Ultrasonic rhinoplasty in Madrid
People looking for ultrasonic rhinoplasty in Madrid often ask whether there is a way of working the bone of the nose with more control. It is a reasonable question, and the answer is more nuanced than a slogan: the technique has possible advantages that have been studied and limits worth knowing. At Clínica EGOS we approach nose surgery as a medical and personal decision, built around your specific case. We have five clinics in the Community of Madrid: Sagasta, Retiro and Castellana in the capital, and Alcobendas and Móstoles outside it.

This page focuses on the technique: what the piezoelectric instrument is, in which parts of the nose it is used, what the studies say, what recovery feels like and what a quote should include.
What is ultrasonic rhinoplasty and how does it differ from the traditional kind?
Ultrasonic rhinoplasty is nose surgery, also known as piezoelectric rhinoplasty, in which the bone is shaped with an instrument that emits high-frequency ultrasound instead of a chisel and mallet. The instrument is designed to cut hard tissue precisely and to affect the surrounding soft tissues, such as skin or mucosa, less.
The difference from traditional rhinoplasty therefore lies in the way the bony part is worked. The aim of the operation is the same: to change the shape of the nose and, when needed, to improve breathing. The tip and the cartilage are worked with the usual techniques, and it is the study of your whole face, not the instrument, that decides the result. The ultrasonic technique can be used in both open and closed rhinoplasty; which one is suitable depends on what needs to be corrected.
In which parts of the nose is ultrasound used?
Ultrasound is used where there is bone. In practice, that means three actions: filing or removing a hump, making the lateral cuts that narrow a wide base, and correcting an asymmetry of the nasal bones. If your main concern is the tip, which is cartilage, the instrument adds little and the surgeon will tell you so plainly.
It is worth understanding this because ultrasonic rhinoplasty is often spoken of as if it were a different operation. It is not: it is the usual rhinoplasty with a different tool for part of the work.
What does the evidence say about ultrasonic rhinoplasty?
It is worth separating what has been measured from what is promised. A systematic review and meta-analysis of 12 randomised trials, from 2024 and published in the Journal of Clinical Medicine, compared piezosurgery with conventional osteotomy. According to its authors, piezosurgery brought less oedema on days 2 and 7 and less ecchymosis (bruising) on days 2, 4 and 7; operating time did not vary.
Those same authors rate the evidence as low quality, and swelling and bruising were measured subjectively. What can honestly be said: there are signs of easier first few days, not a guarantee, and the final result depends on the plan and the surgeon, and varies from person to person.
Who is the ultrasonic technique suited to?
It makes most sense when what you want to change is bone: a hump, a wide base or crooked nasal bones. If what bothers you is mainly the tip, it adds little. The other requirements are those of any rhinoplasty, and the surgeon goes over them with you at the consultation.
Four misconceptions about ultrasonic rhinoplasty
"It is a laser"
It is not. The piezoelectric instrument emits ultrasonic vibrations that cut the bone; it does not burn and does not work like a laser.
"It removes the swelling"
No. There are signs of less swelling and less bruising in the first days, but there will be swelling anyway and the final result takes months.
"It works on the whole nose"
No. It acts on the bone; the tip and the cartilage are worked with the usual techniques.
"The instrument decides the result"
It does not. The plan, made before surgery, and the surgeon's hand decide it.
Questions about the technique to bring to the consultation
Note these down before you go: which part of my nose is bone and which cartilage; for which cuts you would use ultrasound; what difference you expect between using it or not in my case; which part of the operation will be the same as in a traditional rhinoplasty; and how many rhinoplasties with this technique you have performed.
The consultation in Madrid
At the consultation, the surgeon examines your nose inside and out and tells you which part of what you want to change is bone and which is cartilage: whether ultrasound adds anything for you depends on that. The informed consent, with the risks in your case, reaches you in writing before the date, and no result can be promised.
And the rest of the recovery?
Beyond those first few days, recovery is the same as for any rhinoplasty: a splint for the first week, glasses kept off the nose for a few weeks and a final shape that takes months. Your surgeon gives you the timings for your case.

Ultrasonic rhinoplasty at Clínica EGOS is performed by a registered specialist in plastic surgery. What is the price of ultrasonic rhinoplasty in Madrid, and what does the quote include?
Anyone looking for the price of ultrasonic rhinoplasty in Madrid finds very different figures, and with good reason: the cost changes with what has to be done, and correcting only the bone is not the same as also correcting the septum. That is why we do not put a figure up front: the quote is given in writing after the assessment.
A clear quote lists fees, theatre time, anaesthesia, materials, follow-up visits and care. If something is not listed, ask about it before accepting.

Frequently asked questions
What does it mean for a rhinoplasty to be "piezoelectric"?
That the bone is cut with an instrument that vibrates at ultrasound frequencies, instead of a chisel and mallet. It is another way of making the bone cuts, not another operation.
Is it used on the whole nose?
No, mainly where there is bone: hump, lateral cuts and asymmetries of the nasal bones. The tip is cartilage and is worked with the usual techniques; the septum has a cartilage part and a bone part, and the surgeon decides how to treat it.
What tests will I be asked for before surgery?
The ones the surgeon indicates according to your health and age; they will explain them at the first consultation.
Does the operation take longer with ultrasound?
In the 2024 meta-analysis, ultrasound did not lengthen the operation. How long yours takes depends on what needs to be corrected: bone only, bone and tip, or the septum as well.
When can I go back to work?
Every case is different. The surgeon gives you a date at the consultation, allowing for the splint during the first week and for your face staying a little swollen for a few days more.
How much does ultrasonic rhinoplasty cost in Madrid?
It varies with the complexity of the case; after the assessment you receive a written, itemised quote.
Where can I book a first ultrasonic rhinoplasty consultation in Madrid?
We hold consultations at Sagasta, Retiro and Castellana in the capital, and at Alcobendas and Móstoles in the Community; the first assessment takes place at whichever clinic suits you best.
Is ultrasonic rhinoplasty better than the traditional kind?
It depends on what needs to be corrected: if it is mostly bone, it may add something; if it is the tip, it adds little. The design of the nose does not change with the instrument.
