Blepharoplasty in Madrid
Blepharoplasty in Madrid is eyelid surgery that removes excess skin, fat or muscle from the upper eyelids, the lower eyelids or both, to give the eyes a more rested look or to clear the field of vision when the skin is covering it. This page explains how to decide between upper, lower or both, who it is considered for, how the operation and recovery go, what the American Society of Plastic Surgeons (ASPS) says, and what the quote depends on.

Our consultations are in Chamberí, Retiro and Castellana, within the city of Madrid, and in the towns of Alcobendas and Móstoles.
What is blepharoplasty and when is it considered?
Blepharoplasty is both a cosmetic and a functional surgery: it corrects excess skin, fat or muscle in the eyelids. According to the ASPS pages we consulted in October 2026, it can be done on the upper eyelid, the lower eyelid or both, and it serves both to improve the look of the eyes and to solve functional problems of the eyelids.
With the years, drooping eyelids, bags, extra skin and deep dark circles can appear and make your face look tired even when you are not. The ASPS lists what the operation can treat: loose skin that forms folds or alters the contour of the upper eyelid and, at times, gets in the way of vision; fat deposits that show up as puffiness; bags under the eyes; sagging lower eyelids that show the white below the iris; and excess skin with fine wrinkles on the lower eyelid.
It is an outpatient procedure of around an hour, and it is always considered after an in-person assessment.
Blepharoplasty in Madrid: upper, lower or both, and how to decide which one you need
There is no single operation, and getting the area right is the most important part of the consultation.
Upper blepharoplasty
It is considered when excess skin hides the eyelid's own crease or falls over the lashes and gives a tired look, or when it limits the field of vision. The incision follows the crease of the eyelid.
Lower blepharoplasty
It addresses bags or puffiness in the lower eyelid. The incision can go just below the lashes or on the inside of the eyelid, depending on the case.
Combined blepharoplasty
It brings the two together when extra skin above and bags below coincide.
When the problem is not only in the eyelid
The ASPS warns that an upper eyelid that looks droopy may be because the skin of the forehead and the brow has relaxed. And if what has stretched is the muscle that lifts the eyelid, there is a ptosis, which needs a different surgical treatment. That is why the surgeon studies the anatomy of the whole face before proposing anything, and sometimes the solution involves working on the brow as well.
Who is blepharoplasty in Madrid for? Women and men looking for a more rested look
The criteria set out by the ASPS apply equally to women and men: good health, no illnesses that delay healing, a positive attitude with realistic goals and no serious eye problem. The ASPS adds not smoking, and tobacco also harms healing.
There is no fixed age. It is common to have it from the age of 35 or 40, when the signs of ageing in the eyelids start to show, although it can be considered earlier if there is a family history of bags or early excess skin.
The operation does not change the shape of the eyes: it aims to rejuvenate without altering your expression, and if something looks different at first it is because of the swelling, which goes down over time.
It does not correct every dark circle either. Lower blepharoplasty can reduce the bags that contribute to them, but if the problem is darker skin, other treatments may be needed.
Your first blepharoplasty consultation in Madrid
At the first visit the surgeon studies your case, listens to what you expect and prepares a personalised plan, and explains the technique, the expected result and the quote.
Your eyelids and brow are examined and the surgeon assesses how much skin and fat there is in excess, whether there are visual symptoms and which areas should be operated on. It is the moment to say whether you wear contact lenses, whether you have had eye surgery or whether you take medication, and to say what bothers you when you look in the mirror.
Your first assessment can take place at any of our clinics in Madrid.
How the operation goes: anaesthesia, duration and discharge
It is an outpatient procedure. It is usually done under local anaesthesia, although in some cases general anaesthesia may be chosen, and it takes about an hour. No hospital stay is needed: after the operation you do not need to rest at the centre.
The incisions
The surgeon makes them in the eyelids' own creases so that the scar is as hard to see as possible, and removes the excess skin, fat or muscle through them. On the upper eyelid the incision runs along the crease; on the lower eyelid it can go under the lashes or on the inside of the eyelid.
Closing up
The incisions are closed with fine sutures, which dissolve on their own over time or are removed after a few days.
Preparation
Before the operation, the tests that apply according to your health are carried out and you are given some instructions beforehand. Before going into the operating theatre you sign the informed consent required by Law 41/2002.
Recovery from blepharoplasty by stages
These guidelines are general and the surgeon adapts them to each case.
The first days
Swelling, bruising, irritation, dry eyes and some discomfort are normal. According to the ASPS they are managed with cold compresses, a lubricating ointment and the medication the surgeon prescribes, and sometimes the eyes are covered with gauze. The swelling goes down gradually over the first few weeks.
From the first to the second week
Work is usually resumed between seven and ten days. Contact lenses can usually be worn again one or two weeks later, depending on how recovery is going. Stitches that do not dissolve are removed within a few days.
Make-up and sun
It is recommended to wait between ten and fourteen days before putting make-up on your eyes, so that the incisions heal well. The ASPS asks you to protect the area from the sun strictly and to wear very dark sunglasses until healing is complete.
Results: how long they last and how to avoid an "operated" look
Results usually last many years, although ageing carries on and the eyelids keep changing.
What we aim for is a more rested look, not a different one. That is why the plan is designed with you: how much skin to remove, what to do with the fat and whether the brow needs work. A discreet result is one where nobody can tell there was an operation.
Blepharoplasty for functional reasons and the Spanish Social Security
When excess skin on the upper eyelid blocks the field of vision, the operation can improve sight as well as appearance. This is the situation the ASPS describes when it talks about skin that at times gets in the way of vision.
As for public healthcare, Royal Decree 1030/2006 (the common portfolio of services of the health system) does not include operations done for cosmetic purposes. If a drooping eyelid limits your vision, that is a medical matter that the health service of your region can assess; that decision does not depend on us.
Can it be combined with other facial operations?
Yes. Blepharoplasty is often combined with a facelift when the face is also to be treated, or accompanied by work on the brow when it is the brow that is pulling the eyelid down.
Combining surgeries lengthens the time in the operating theatre and makes the postoperative period somewhat more complicated; that is why the surgeon assesses which ones are worth doing together and which to leave for another time.
What to write down before the consultation
Bring a note of whatever you want to know about recovery: whether there will be a dressing or gauze and when it is removed, whether the stitches dissolve or are taken out, when you can go back to work and to exercise, when the first check-up is and what the quote includes.
Who performs blepharoplasty in Madrid?
Registered plastic surgeons at Clínica EGOS assess your case and propose the technique; the same medical team handles both the assessment and the operation.
Before you book a date, we tell you in writing who will operate on you, with their registration number with the medical association, and at which centre, with its health registration number.
The medical association lets you verify that registration number, and the health register lets you verify the centre.
What the quote for blepharoplasty in Madrid depends on
The figures that go around are very mixed because the cost depends on several things, and none of them can be relied on without seeing the eyelids. That is why the quote arrives in writing after the first consultation.
The main factors are these: whether the operation is upper, lower or both; whether it is combined with other facial rejuvenation techniques, such as a facelift; the type of anaesthesia; and whether the brow also needs treating.
It should itemise the prior assessment, what corresponds to the surgeon and the anaesthetist, the operating theatre with the materials and the follow-up check-ups. We give it to you in writing, itemised.
Before deciding, check that two quotes include the same operation, the same anaesthesia and the same check-ups.

Frequently asked questions
How much does blepharoplasty cost in Madrid and what does the quote depend on?
It arrives in writing and itemised after the consultation, and it depends on whether the upper eyelids, the lower eyelids or both are operated on, on whether it is combined with other techniques and on the anaesthesia. To compare quotes, make sure they cover the same things.
Does Social Security cover blepharoplasty?
If the reason is cosmetic, no: Royal Decree 1030/2006 does not include those operations. If the skin of the eyelid limits your vision, that is a medical matter that your health service can assess.
How many years does the result of an upper blepharoplasty last?
It usually lasts many years, although the eyelids keep ageing. How long it lasts in your case depends on your age, your skin and how it changes over time.
Which one do I need: upper, lower or both?
It depends on where the excess is. Upper treats the extra skin above, lower treats the bags and combined treats both. The surgeon decides when examining the eyelids and the brow.
At which centre and with which surgeon?
Registered plastic surgeons at Clínica EGOS operate. Before you decide on a date, you receive in writing the name and medical association registration of whoever operates and, in addition, the centre and its health registration.
How long does recovery take? When can I drive or wear contact lenses?
Most people go back to work between seven and ten days, and to contact lenses one or two weeks later. As for driving, the surgeon tells you when it is safe in your case.
From what age is it usually considered?
There is no fixed age. It is common from the age of 35 or 40, although it can be considered earlier if there are bags or excess skin at an early age.
What is the first consultation like and which clinic should I book at?
Your case is studied and you are told which technique fits, what result to expect and what the quote includes, with no commitment to operate. You can book at any of the Madrid clinics: the choice depends on how close it is to your home or work.
