Buttock augmentation in Barcelona

Anyone searching for buttock augmentation in Barcelona finds two different things on the same pages: operations that go through an operating theatre and fillers that do not. It is worth knowing this from the start, because they are not the same thing and are not approached in the same way. At Clínica EGOS we are talking about surgery: increasing the volume and projection of the buttocks with fat from your own body, depending on what the examination shows.

Buttock augmentation in Barcelona · Clínica EGOS

On this page we tell you how the technique is decided, what is done step by step in a gluteal fat transfer, what recovery is like, who operates on you and what you should ask to see in the quote, so that you arrive at the consultation with your questions in order. We have five clinics in the city and you can start with Balmes, the cosmetic surgery clinic in Barcelona; the others are Aribau, Guinardó, Madrazo and Poblenou.

What is buttock augmentation and how is it different from a filler?

Buttock augmentation, also called augmentation gluteoplasty, is a plastic surgery procedure that gives the buttocks more volume, shape and projection. The American Society of Plastic Surgeons (ASPS), whose website we consulted in October 2026, says it is done with implants, with fat grafting or, at times, with a mix of the two. Fat grafting is what is commonly known as "BBL", from the English initials of Brazilian butt lift.

Injectable fillers are something else. They are not done in an operating theatre or with fat from other areas, the body ends up reabsorbing the product, and they are meant for moderate volume touch-ups, not for the change someone considering an operation is looking for. On this page we talk only about surgery.

And what about a buttock lift?

No, that is not the same either. The ASPS distinguishes augmentation from a buttock lift, which removes loose skin and tissue and improves texture and tone, but does not add volume. Which of the two operations, or which combination, suits you is decided at the consultation. If what you want is to reduce and define, not to enlarge, that is liposuction, sometimes called liposculpture: you will find it on our liposuction page.

What EGOS offers you if you decide to have surgery in Barcelona

Before comparing clinics, look at what you can check about each one. About ours, this:

Our own medical team

The assessment, the operation and the follow-up checks are all carried out by the same EGOS medical team, and the plan is put in writing before you decide anything.

Where to have your visits

For the first assessment you can choose from Balmes, Aribau, Guinardó, Madrazo and Poblenou the one that is most convenient: we hold consultations at all five.

Your own fat: how the technique is decided

The choice depends on your body and on what you want to achieve, not on a trend. At EGOS we carry out augmentation with your own fat.

Fat transfer (fat grafting)

Fat is taken from other areas, such as the abdomen, thighs or hips, by liposuction; it is prepared and injected into the buttocks to give shape and volume. It has an advantage: because fat is taken from other areas, the silhouette is also contoured. In return, you need to have fat available, and the change in volume has a ceiling set by the amount there is and the amount that is retained. It is the option for someone who has donor areas and wants a result made with their own tissue.

Buttock implants are a different technique, which we do not cover on this page.

The decision, in three questions

How much fat you have and where it can be taken from; how much volume you want to add; and what proportion your body keeps between waist, hip and thigh. With your measurements in front of them, the surgeon tells you what is possible and what is not. Sometimes the answer is that it is not the right time.

Are you a candidate? The first buttock augmentation consultation in Barcelona

According to the ASPS, the person considering buttock augmentation is someone in good health with realistic expectations and one of these situations: they feel their buttocks are small, are not happy with their roundness or shape, or have asymmetry. The ASPS adds a criterion that applies to any operation: it is done for yourself, not to meet someone else's wishes.

The first visit starts with what you want. Then the surgeon reviews your medical history, your medication and your previous operations, measures your body, takes photographs for your medical record and explains the options. In the case of your own fat, they also assess where there is tissue that can be taken. If there is a condition that advises against it, they say so. And if the best thing is to wait, they say that too.

Tell the surgeon everything, even if it does not seem important: what you say changes the plan.

Buttock augmentation with your own fat, step by step

Where the fat is taken from

The usual areas are the abdomen, the flanks, the back and the thighs. The ones chosen are those with the most fat and those that, by being left with less, improve the figure. That liposuction is part of the operation and also defines the contour of the waist and hips.

How the fat is prepared

It is washed and separated from fluids and debris before it is used. It is not an injection of just any material that has been removed: it is treated so that it can be grafted.

How it is injected

It is done in small amounts, with fine cannulas and from several points, spread in layers to shape the form. The fat is placed in the subcutaneous tissue, above the muscle, as the working group of several international plastic surgery societies recommended in 2018.

How much fat is retained

Part of the grafted fat does not survive, which is why the immediate volume is not the final one. According to the ASPS, it can take up to a year before the final result is seen after fat grafting surgery, because the body reabsorbs part of what was injected. That fat also behaves like any other fat in your body: if you put on weight it may increase, and if you lose weight it may decrease.

How the shape and size are planned

A buttock augmentation is not measured in cubic centimetres chosen in advance, but in proportions. At the consultation, the surgeon looks at your body standing, from the front, from the side and from behind, and assesses the relationship between waist, hip, buttock and thigh. With that reference they decide how much volume to add, where, and whether it is worth also defining the waist or flanks with liposuction in the same operation.

Bringing photographs of silhouettes you like helps the surgeon more than a number does, and so does saying what you do not want. What you see in a photo of another person is not a promise for your body: your bone structure, your skin and your fat distribution are your own. The surgeon explains which part of what you are looking for is possible and which part is not.

It is worth talking about the sport you do and the time you spend sitting. If you work in an office or drive a lot, you need to plan what the post-operative period will be like, because sitting is exactly what has to be avoided at first.

Where and how the operation takes place

At EGOS, buttock augmentation is done under general anaesthesia and lasts about three hours. We also advise spending at least one night in a hospital centre.

According to the ASPS, buttock augmentation can take place in an accredited operating theatre, an ambulatory surgery centre or a hospital. If you are operated on without an overnight admission, someone should take you home and stay with you at least that night.

Before the date, a blood test and whatever studies your case calls for are requested. Among the preparations the ASPS lists are reviewing your usual medication, not smoking and stopping some medicines that increase bleeding. For any surgical procedure, Spain's Law 41/2002 on patient autonomy requires written consent (art. 8.2).

Recovery: compression garment, sitting, sleeping and sport

Timelines vary and the team fine-tunes them at each check-up. This is what is usually explained.

The first days

When the operation ends, dressings and a compression garment are applied, and according to the ASPS you do not take it off, even at night, for as long as the surgeon says. In these days it is best to lie face down, on your side or standing, and to sit as little as possible. Our advice is to sit as little as possible for the first 72 hours and, after that, to sit with your weight on the back of your thighs.

The first weeks

Sitting carefully remains the key point: the ASPS advises avoiding sitting for long periods for about two weeks, and some surgeons prefer longer, because pressure damages the fat. To sit, a cushion behind your thighs lifts the buttocks and avoids resting on what has been grafted. Sleep face down or on your side, as the team indicates. You may be recommended a lymphatic massage with a qualified professional.

Return to normal activities is estimated at between two and four weeks, avoiding strenuous effort for at least six weeks.

Sport

The ASPS notes that, in general, you can return to exercise from six to eight weeks. The surgeon decides when in your case.

The result

The shape is visible from the first moment, but the swelling takes time to go down. At around three to six months the shape can be seen, and if fat has been used the volume may keep settling for up to a year. The ASPS adds that scars can take up to two years to mature.

How to maintain the result

According to the ASPS, the result is long-lasting as long as you maintain a stable weight and good fitness, although it is normal to lose some firmness with age. If fat has been used, weight matters especially, because that fat responds to weight changes like the rest. That is why the surgeon will ask you to come to the operation at a stable weight and to maintain it afterwards.

Who will operate on you in Barcelona?

The operation is carried out by a registered plastic surgeon at Clínica EGOS, from the same team that assesses you at the consultation and follows you through the check-ups.

Before you book a date, we tell you in writing who will operate on you, with their registration number with the medical association, and at which centre, with its health registration number.

Price of buttock augmentation in Barcelona: what it depends on and what it includes

The price of a buttock augmentation in Barcelona is searched for a lot and varies widely, which is understandable: it does not cost the same to take fat from one area as from three, or to treat a small volume as a large one. That is why the figure comes after seeing you, in a written quote once the assessment has been done.

What it depends on

How many areas need liposuction and how much fat is needed, the anaesthesia and the overnight stay, and whether or not another surgery is added.

What it should include

A clear quote itemises the fees, the operating theatre, the anaesthesia, the stay, the compression garment and the check-ups. We give it to you in writing, itemised.

Frequently asked questions

How much does a buttock augmentation in Barcelona cost?

It changes with the areas the fat comes from, with the volume treated, and with the anaesthesia and the overnight stay; that is why we talk about figures only after the assessment, with a written, itemised quote.

Is fat transfer the same as an injectable filler?

No. Fat transfer is an operation using your own fat; the filler does not involve an operating theatre, the body ends up reabsorbing it, and it is used for small touch-ups.

How much fat is needed and where does it come from?

It depends on the volume you want to add and on what you have available. It is usually taken from the abdomen, flanks, back or thighs, in the areas where liposuction also improves the contour.

How long do the results last and how much of the fat is retained?

Part of the grafted fat is reabsorbed in the first months, and the fat that stays behaves like the rest of your fat. The result is long-lasting if you maintain a stable weight, but the body keeps changing with age.

When do I see the final result?

The change is visible straight away, but the swelling needs to go down: the shape can be seen at around three to six months, and if fat grafting was done the volume may keep settling for up to a year.

Will there be scars?

They are small and are in the areas where the fat is taken from and placed, in places that are barely visible. How each skin heals varies.

When can I sit down after the operation?

In the first few days, as little as possible; after that, with a cushion behind your thighs so you do not rest on the buttocks. Long periods of sitting are avoided for about two weeks or more, depending on the surgeon.

How do you sleep after a buttock augmentation?

Face down or on your side, as the team tells you, and never resting your weight on the operated area during the first weeks.

When do I go back to work and sport?

A desk job requires planning how to sit; normal life returns in two to four weeks, and sport, according to the ASPS, from six to eight weeks. The surgeon sets the dates for you.

Can it be combined with liposuction or other surgery?

Yes, liposuction is part of fat transfer, and augmentation is sometimes combined with abdominal correction or a thigh lift. How many things at once is a decision for the surgeon.

What if I get pregnant afterwards?

It does not rule it out, but the weight changes that come with it can alter the volume of the buttocks, especially with fat. If you are thinking of having children, mention it at the consultation.

Is it covered by Spanish Social Security?

No, if the purpose is purely aesthetic: the public healthcare portfolio set by Royal Decree 1030/2006 does not include these procedures, unless there is a medical cause behind them.
